•  A copy of the child's birth certificate 

    OR 

• Other acceptable Proof of the child's identity** WITH a Notarized affidavit*** 

  ** Examples: Hospital Certificate, I94, passport, State photo ID, baptismal certificate, etc. 
*** Notary service is available at the Registration Center for those registering
